The honest comparison
Kindle Create is Amazon's free desktop formatter, and if you're certain your book will only ever be a Kindle exclusive, it does the job. The catch is its output: a .kpf file that can be uploaded to KDP and nowhere else. There's no EPUB to take to Apple Books, Kobo, Google Play or an aggregator — going wide later means formatting the whole book again in a different tool.
doc2epub produces the portable version of the same result: a standard EPUB 3, validated with epubcheck, that KDP accepts directly (it has since 2021) and every other store accepts too — plus the print-ready PDF interior Kindle Create doesn't give you a portable form of. It runs in the browser, so there's nothing to install, and you preview the complete typeset book before paying.
